Q1: What kind of company is VIVA VIDA!?

A: VIVA VIDA MEDICAL LIFE CO., LTD. IS THE FIRST AND ONLY LIFE AND
    HEALTH INSURANCE COMPANY FOR FOREIGNERS LIVING IN JAPAN
    REGISTERED BY THE GOVERNMENT. [Registration No. 51(Small Amount and
    Short Term Insurance) by the Chief of the Kanto Local Finance Bureau]

Q2: I did not join the national health insurance, can I join VIVA VIDA!?

A: VIVA VIDA! is used as a safety net for people with no public insurance. If you did
    not join any health insurances, please join us soon.

Q3: I don't speak Japanese, can I inquire about the procedure?

A: We have staff that speaks in Japanese, English, Filipino, Portuguese, Spanish and
    Chinese.

Q4: At present I have an existing illness, can I still join VIVA VIDA!?

A: You can join VIVA VIDA! but the existing illness and other illness from the past will
    not be covered.

Q5: I don't have relatives in Japan to be beneficiaries of my life insurance, can my relatives in my home country receive the benefits?

A: Relatives living in your home country can be your beneficiaries.

Q6: Can I pay the premium in installments?

A: Premium can be paid in installments up to six installments

Q7: I am a Japanese, can I join VIVA VIDA!?

A: Japanese or foreigners with permanent resident status overseas can join VIVA
    VIDA!.

Q8: Can a 0 year old or newly born baby join VIVA VIDA!?

A: We regret that 0 year old or newly born baby cannot join. VIVA VIDA! accepts ages
    from 1 to 55 years old.

Q9: I have sent my application, when will be the effective date of the insurance coverage?

A: After receiving the application, an evaluation is done, the next day after the day of
    payment will be the start of the coverage. After the evaluation, the billing form will
    be sent to the member.

Q12: I am living in Japan but went abroad, if I have an injury or illness during my stay abroad will it be covered?

A: VIVA VIDA! covers only illness and injuries that happen in Japan.

Q13: If the medical treatment cost for one illness/injury is from JPY 10,000 and above will the whole amount be reimbursed?

A: VIVA VIDA! will reimburse the whole amount as long as it is from JPY 10,000 and
    above (conversion of JPY 10/ 1 medical treatment fee point). Example: when total
    medical cost is JPY 12,000, we will reimburse the whole amount.

Q14: Is sending the VIVA VIDA! medical diagnosis report necessary to receive a claim? Is the payment for the claim forms be reimbursed?

A: VIVA VIDA! will reimburse and pay the fee up to JPY 5,500.

Q15: I have sent my claim form to VIVA VIDA!, how long will it take to process my claim?

Q16: Can I make claims for my medical expenses even when I haven't fully paid my premium?

A: Yes, but please ensure to pay the monthly premium before claiming any
    reimbursement.

Q17: Is regular medical check-up included in the coverage?

A: Only medical expenses when illness or injury occurred will be reimbursed.

Q19: Why is the dental treatment will not be covered?

Q20: If I have no claim in one year, will I be able to collect a certain amount of rebate?

A: VIVAVIDA! is a Small Amount and Short Term Insurance Company, there is no
    refund or rebates.

Q21: Is it an object of deduction of life insurance of the year end adjustment?

A: VIVAVIDA! is a Small Amount and a Short Term Insurance Company, the premium
    deduction in the tax code is not applied.
